Different types of services and providers have varying degrees of availability, and not all providers are able to accept all insurance plans; therefore, before you complete the New Client Inquiry Form below, we encourage you to check the status of current potential availability for the services you may be seeking by clicking HERE and we recommend that you review our FAQ for some helpful information!
Please note - although we truly wish that we could serve every client that reaches out, completion of this form and/or submission of a referral from your child’s pediatrician does not guarantee that we will have openings available; you will be provided with referrals in the event we cannot serve you at this time.

Although it may seem like a lot of information, the details that you provide on the new client inquiry form will assist our team in identifying if we have the staffing for the services you are seeking and help us to match you with the best provider for your needs! Please provide as many details as you feel comfortable sharing.
The administrative team reviews new client inquiries twice per week; therefore, to allow adequate time for case review, please expect a reply from the intake coordinator VIA EMAIL within 2 to 5 business days. Please check your spam folder and/or reach out to Info@BeaconPediatric.com if you have not heard from us within this time frame of if you have any questions!
